How much does it cost and what are the requirements?

The cost register for an Import/Export License is R2350

CIPC company registration certificate (COR14.3)
Proof of Banking Details (Original stamped letter from the bank, or an original bank statement - neither should be older than three months).
Letter of Accountant with a Practice number ( We will provide )
ID Copies for all company directors
Proof of business address ( Not older than 2 months and should be within the borders of SA )
